The immunization office is open from 9 a.m. to noon and 1 to 4 p.m. Monday through Friday and can be reached by calling 623-876-7944.

Required student immunizations include: DTAP/DTP (diptheria, tetanus and pertussis), IPV/OPV (polio), MMR (measles, mumps and rubella), hepatitis B, varicella (chicken pox vaccine or history of disease, a requirement at some grade levels), in addition to the Tdap and meningococcal requirements for incoming sixth-graders.

Students may also receive free immunizations at an event provided by the Maricopa County Department of Public Health from 9 a.m. to 2:30 p.m. Aug. 2. The Maricopa County event will be at various locations throughout the Phoenix area.
